CREDIT CARD PAYMENT AUTHORIZATION & SERVICE ACCEPTANCE AGREEMENT

Authorization for Services

I, the undersigned customer, authorize ESG NATIONAL LLC to perform the plumbing services described in the estimate, invoice, or work order provided to me. I acknowledge that:

• I requested the services to be performed.
• The scope of work, pricing, and any applicable fees have been explained to me before work began.
• I have had the opportunity to ask questions regarding the services and charges.
• I voluntarily authorize ESG NATIONAL LLC to charge my credit card for the total amount shown on the final invoice.

Completion of Work

I acknowledge that the authorized services have been completed as agreed. I have inspected the completed work, or have chosen not to inspect it, and I accept the services as completed. I understand that any warranty offered by ESG NATIONAL LLC applies only as stated in writing and does not cover conditions outside the agreed scope of work.

Payment Authorization

A 4% processing fee will be added.

I authorize ESG NATIONAL LLC to process my credit card for the total amount due (including 4% processing fee).

I certify that I am the authorized cardholder or have authority to use the payment method provided.
